Documents needed for registration:
1. Click, download and complete the Player Code of Conduct
2. Click, download and complete the Consent to Treat:Medical History
3. Go to www.usahockeyregistration.com to obtain your USA Hockey Number. Print out the confirmation pager.
4. Click, download and complete the 2012-2013 Player Registration Form.
5. Click, download and complete the liability waiver.
Bring all of the above, along with payment to one of the scheduled registration days.
